By DEAN REA/Correspondent/The Herald — The second of three COVID-19 vaccine clinics is scheduled at Pleasant Hill High School on Friday, Oct. 1, as school personnel prepare to meet state vaccination requirements.
Friday’s clinic is scheduled between 3:30 p.m. and 6 p.m. at the student center. A clinic also is scheduled there on Oct. 22.
After Oct. 18, teachers, staff and volunteers may not teach, work, learn, study, assist, observe or volunteer at a school unless fully vaccinated or can provide proof of religious or medical exception.
Under Oregon law, minors 15 years of age and older may consent to medical treatment, including vaccinations, when provided by a physician, physician assistant, naturopath, nurse practitioner, dentist or optometrist, or other professionals operating under the license of these providers.
This consent form is intended to remove vaccine access barriers for individuals 12- to 14-year-olds by allowing them to get vaccinated without parental or guardian accompaniment.
If an individual age 12, 13 or 14 is not accompanied by a parent or legal guardian, the youth must have a signed copy of this consent form (Español) or will not be eligible to receive the vaccine.
